Key takeaways
- Ranked by human evidence and legality, the best HGH peptide is tesamorelin, then sermorelin. They are the only two a US clinician can prescribe and a licensed pharmacy can fill in October 2026.
- Tesamorelin has an FDA approval and an 806-patient trial program: about 15% less visceral fat in 26 weeks. Sermorelin has small 1990s trials showing it restores growth hormone and IGF-1 in older men at 1 to 2 mg a day, and a lower price.
- MK-677 has the best body-composition trial of the rest (about 1 kg more fat-free mass, no strength gain, higher glucose), but it is not a peptide and cannot be compounded. CJC-1295 and ipamorelin have hormone studies only and no legal supply.
- None of them has been shown to slow aging, build usable strength or replace growth hormone in someone who is truly deficient. The right answer for many readers is none.
The best HGH peptides, ranked by what has been shown in humans and what a licensed US pharmacy may dispense, are tesamorelin first and sermorelin second; MK-677, CJC-1295 and ipamorelin follow, in that order, as products no pharmacy may compound. None is growth hormone, and none has been compared with it. Our HGH peptides vs HGH hub explains how secretagogues differ from somatropin and why HGH itself is off the table for anti-aging; sermorelin vs HGH covers the most common head-to-head question.
The ranking
| Rank | Compound | Evidence grade (main claim) | Key human result | Legal status, October 2026 | Prescribable | Monthly cost |
|---|---|---|---|---|---|---|
| 1 | Tesamorelin | A in HIV lipodystrophy; B outside HIV | 806 adults, 2 mg daily for 26 weeks: visceral fat -15.4% versus placebo | FDA-approved (Egrifta, Egrifta WR); brand only, not compoundable because it is a biological product | Yes | $2,400 to $2,800 brand |
| 2 | Sermorelin | B for GH and IGF-1; C for outcomes | 10 men aged about 68, 1 mg twice daily for 14 days: GH and IGF-1 restored to young-adult levels | Formerly approved (Geref); compoundable | Yes | $150 to $400 |
| 3 | MK-677 (ibutamoren; not a peptide) | B | 65 adults aged 60 to 81, 25 mg daily: fat-free mass +1.1 kg versus -0.5 kg at one year, no strength gain | Category 2; not compoundable | No | No legitimate price |
| 4 | CJC-1295 | B for GH and IGF-1; D for outcomes | One injection raised GH 2- to 10-fold for 6+ days in healthy adults | Nominated but withdrawn; committee voted against; not compoundable | No | No legitimate price |
| 5 | Ipamorelin | C for GH; D for outcomes | 117 adults after bowel surgery: no benefit over placebo | Nominated but withdrawn; committee voted against; not compoundable | No | No legitimate price |
We ranked on four things, in order: the strength of the human evidence for the claim each is sold on (grades A to D per our editorial policy), whether a licensed pharmacy can legally dispense it, the safety record, and cost from our October 2026 price survey. Popularity did not count, which is why the most-marketed stack sits at the bottom.
1. Tesamorelin: the one with real trials
Tesamorelin is a stabilized copy of the full 44-amino-acid growth hormone-releasing hormone and the only secretagogue with a current FDA approval. In two pooled phase 3 trials, 806 adults with HIV took 2 mg a day or placebo for 26 weeks: visceral fat fell 24 cm2 against a 2 cm2 rise, a treatment effect of -15.4%, with subcutaneous fat unchanged and IGF-1 up 108 ng/mL; patients who continued to 52 weeks kept a 17.5% reduction (Falutz 2010). Outside HIV there is one trial, 60 abdominally obese adults with low growth hormone output, with a 12-month treatment effect of -35 cm2 and no change in glucose (Makimura 2012). The fat returns when you stop, not everyone responds, and it raises IGF-1 enough that the label requires monitoring. It is the right pick when visceral fat is the measured target; see the tesamorelin profile for the label data and the HGH peptides hub for how it compares with GH.
2. Sermorelin: the prescribable nudge
Sermorelin is the first 29 amino acids of GHRH, once approved as Geref for children with GH deficiency and still compoundable because of that history. In 10 healthy men aged about 68, 1 mg injected twice a day for 14 days restored 24-hour growth hormone and IGF-1 to the levels of men in their twenties, while 0.5 mg twice a day did not (Corpas 1992). In 11 men aged 64 to 76, 2 mg once nightly for six weeks raised overnight GH but not IGF-1, improved two of six strength measures and changed nothing on DEXA (Vittone 1997). That is grade B for hormones and C for anything you would notice. Most clinics prescribe 200 to 500 mcg a night, well below those doses, so ask what dose your prescriber expects to move IGF-1; the sermorelin profile and the peptide calculator help with the arithmetic.
3. MK-677: best data of the rest, no legal supply
MK-677 (ibutamoren) is an oral small molecule that activates the ghrelin receptor; it is sold with peptides but is not one. Its trial is the longest placebo-controlled study of any secretagogue in healthy adults: 65 adults aged 60 to 81 took 25 mg or placebo daily for two years. Growth hormone and IGF-1 rose to young-adult levels and fat-free mass rose 1.1 kg against a 0.5 kg fall, but weight rose 2.7 versus 0.8 kg, fasting glucose rose about 5 mg/dL, insulin sensitivity fell, and the extra mass produced no change in strength or function (Nass 2008). FDA has kept it in Category 2 since September 2023 (FDA categories 2026), and its advisory committee voted against it in October 2024. The muscle-growth guide has the later trials, including a heart-failure signal.
4. CJC-1295: strongest on paper, untested where it matters
CJC-1295 is a modified GHRH(1-29); its DAC form binds albumin and lasts days. In two placebo-controlled trials of 28 and 49 days in healthy adults, one injection raised mean growth hormone 2- to 10-fold for six days or more and IGF-1 1.5- to 3-fold for 9 to 11 days, with a half-life of 5.8 to 8.1 days (Teichman 2006). No outcome was measured, the version without DAC sold for nightly use has no published human study, and the longest controlled exposure is seven weeks. FDA now lists it as nominated but withdrawn (FDA categories 2026), after its committee voted against every form in December 2024; no pharmacy may compound it. Details are in the CJC-1295 and ipamorelin profile.
5. Ipamorelin: selective in animals, failed its one trial
Ipamorelin is a five-amino-acid ghrelin-receptor agonist marketed for releasing growth hormone without raising cortisol, a property shown in animals. Its only efficacy trial gave 117 adults intravenous ipamorelin 0.03 mg/kg or placebo twice daily after bowel surgery: median time to a tolerated meal was 25.3 versus 32.6 hours, not a significant difference (p = 0.15) (Beck 2014). No study has measured fat, muscle, sleep or recovery on it, or what the 100 to 300 mcg subcutaneous doses clinics sell actually do. Like CJC-1295 it is listed as nominated but withdrawn, its committee vote was 0 yes and 12 no, and it cannot be compounded. The combination with CJC-1295 has never been tested in any trial; the four-way comparison has the full record.
Where growth hormone itself fits
Somatropin is not on this list because it is not a secretagogue, and because distributing it for anything other than an FDA-approved indication, such as adult GH deficiency confirmed by testing, is a federal felony. If you have pituitary disease or suspect true deficiency, the route is an endocrinologist and a stimulation test, not a peptide; the hub explains the diagnosis and sermorelin vs HGH compares the two directly.
Who should pick none
Every compound above raises growth hormone and IGF-1, so the same people should skip all of them, or use one only with a specialist's sign-off:
- anyone with an active cancer or a cancer history their oncologist has not reviewed, since IGF-1 is a growth signal and active malignancy is a labeled contraindication for tesamorelin;
- anyone with diabetes, prediabetes or diabetic retinopathy without close monitoring; MK-677 raised fasting glucose and lowered insulin sensitivity in its two-year trial, and tesamorelin requires glucose monitoring on its label;
- anyone pregnant, trying to conceive or breastfeeding;
- anyone with pituitary disease, a pituitary or brain mass, or past pituitary surgery or head radiation, who needs testing rather than a secretagogue;
- tested athletes, since all are prohibited in sport;
- anyone whose real goal is weight loss (none lowers weight; see best peptides for weight loss), longevity (none has evidence), or muscle (training and, if deficient, testosterone do more);
- anyone unwilling to have IGF-1 and glucose measured before starting and at 8 to 12 weeks, because without those numbers nobody can tell whether the drug is doing anything.
Bottom line
The honest ranking has two prescribable entries. Tesamorelin has the evidence, the approval and the price; it earns that price when visceral fat is the measured target. Sermorelin is the cheaper, feedback-preserving option with small, old trials and a dose gap worth asking about. MK-677 has better data than either of the injectable research peptides and the same legal problem. CJC-1295 and ipamorelin have hormone studies, no outcome data and no legal supply. If you want an HGH peptide because you expect it to make you younger, the best choice on the evidence is none.
Frequently asked questions
+What is the best HGH peptide?
By evidence, tesamorelin: it is FDA-approved, and in 806 adults with HIV-associated belly fat 2 mg a day cut visceral fat about 15% in 26 weeks. By cost and simplicity, sermorelin: small trials show 1 mg twice a day restores growth hormone and IGF-1 in older men, and compounded it runs about $150 to $400 a month. Both can be legally prescribed. Nothing else sold as an HGH peptide can be compounded in the United States in October 2026.
+What is the strongest HGH peptide?
On hormone levels alone, CJC-1295 with DAC: one injection raised growth hormone 2- to 10-fold for six days or more and IGF-1 1.5- to 3-fold for 9 to 11 days in healthy adults. Strongest is not best. That sustained stimulus has been tested for at most seven weeks, has no outcome data and cannot be compounded. Tesamorelin, which raised IGF-1 by about 108 ng/mL in its trials, is the strongest one you can be prescribed, and its label requires IGF-1 monitoring for that reason.
+What are the best HGH peptides for muscle growth?
None has been shown to build muscle a person can use. MK-677 added about 1 kg of fat-free mass over a year in older adults with no gain in strength; tesamorelin added about 1 kg of lean mass in HIV trials without any strength measurement; sermorelin's small trials found two of six strength tests improved in older men. Growth hormone itself did not improve strength in healthy young adults. For muscle, our muscle-growth guide ranks the options, and the compound with real evidence is testosterone in men who are deficient.
+What is the best peptide for HGH production?
If the goal is to raise your own growth hormone output with the natural pulse preserved and a prescription you can legally fill, sermorelin taken at bedtime is the most direct choice; the studied adult doses were 1 to 2 mg a day. If the goal is a measured effect on visceral fat, tesamorelin. Either way, the test of whether it is working is a repeat IGF-1 at 8 to 12 weeks, not how you feel.
+Are HGH peptides worth it for men over 40?
Only for a specific reason. Growth hormone output falls with age in everyone, and no trial has shown that raising it in healthy middle-aged men improves strength, fat, energy or longevity. A man with documented visceral fat out of proportion to his weight, or a symptomatic low IGF-1, has a case for tesamorelin or sermorelin under monitoring. A man with low testosterone needs that treated first, and a man with pituitary disease needs an endocrinologist, not a peptide.
+Why isn't CJC-1295 with ipamorelin ranked higher?
Because it has the least evidence and no legal supply. CJC-1295 has two hormone studies lasting up to 49 days; ipamorelin has a pharmacokinetic study and one surgical trial that did not beat placebo; the combination has never been tested in any trial. FDA's advisory committee voted against both in 2024, and no licensed US pharmacy may compound either in October 2026.
+Can I buy HGH peptides without a prescription?
Not legally from a pharmacy. Sermorelin and tesamorelin require a prescription, and CJC-1295, ipamorelin and MK-677 cannot be compounded at all. Websites that sell them as research chemicals with dosing instructions are selling unapproved drugs, which FDA said in its 2026 warning letters; the vials have no label, no pharmacist and no verified content.
