HGH Peptides vs HGH: Secretagogues Explained, Legality, Which Can Be Prescribed

HGH peptides are not HGH. How growth hormone secretagogues differ from somatropin, what each has been shown to do in humans, why HGH for anti-aging is a federal crime, how adult GH deficiency is diagnosed, and which options a US clinician can legally prescribe in October 2026.

Key takeaways

  • "HGH peptides" are growth hormone secretagogues: molecules that make your own pituitary release growth hormone (GH). HGH itself is somatropin, a recombinant copy of the hormone that replaces it directly. Different drugs, different evidence, different law.
  • Somatropin is FDA-approved for adult GH deficiency confirmed by testing, several pediatric growth disorders, HIV wasting and, in one brand, short bowel syndrome. Distributing it for anti-aging, bodybuilding or sport is a federal felony under 21 U.S.C. 333(e), punishable by up to five years in prison.
  • Secretagogues keep the feedback loop: IGF-1 and somatostatin still cap the pituitary's output, which limits both the risk and the effect, and they need a working pituitary. Injected GH sets the level itself.
  • In healthy older adults, GH moved about 2 kg from fat to lean mass, improved no function, and caused more edema, joint pain, carpal tunnel syndrome and glucose problems. No secretagogue has beaten that on outcomes; none has been compared with GH head to head.
  • In October 2026 a US clinician can prescribe two secretagogues, tesamorelin and sermorelin. MK-677, CJC-1295 and ipamorelin cannot be compounded. Anyone with active cancer, diabetic retinopathy, a pregnancy or an unevaluated pituitary problem should take none of them.

Search for "HGH peptides" and you get a menu: sermorelin, tesamorelin, ipamorelin, CJC-1295, MK-677, sold as a way to get what growth hormone does without a growth hormone prescription. This page tests that claim against growth hormone itself: what GH is approved for, why it is policed like a controlled substance, how the secretagogues differ, and which of them a licensed clinician can prescribe. For a ranking, see best HGH peptides; for the single most common comparison, see sermorelin vs HGH; and for the secretagogues against each other, the four-way comparison.

What growth hormone is, and what it is approved for

Growth hormone is a 191-amino-acid protein made by the pituitary. Somatropin is the recombinant version, with a sequence identical to the natural hormone (Genotropin label 2026). It acts on GH receptors throughout the body and drives the liver to make IGF-1, which carries most of GH's effects on muscle, fat, bone and growth.

Its approved uses are narrower than the marketing suggests. Across the current somatropin labels on DailyMed, they are:

  • Children: growth failure from GH deficiency, Prader-Willi syndrome, Turner syndrome, small for gestational age without catch-up growth, idiopathic short stature (height at or below -2.25 standard deviations), and, depending on the brand, SHOX deficiency or Noonan syndrome.
  • Adults with GH deficiency, either adult-onset (pituitary or hypothalamic disease, surgery, radiation or trauma) or persisting from childhood. The label adds that confirming the diagnosis involves a GH stimulation test, except in patients with several other pituitary hormone deficiencies from organic disease or a congenital or genetic cause (Genotropin label 2026).
  • HIV with wasting or cachexia (Serostim), to increase lean mass and weight and improve endurance, alongside antiretroviral therapy, at 0.1 mg/kg a day up to 6 mg (Serostim label 2026).
  • Short bowel syndrome in patients on specialized nutritional support (Zorbtive), a four-week course alongside diet and glutamine (Matarese 2005). Zorbtive was approved in 2003 and has no current label on DailyMed.

Aging, body composition in healthy adults, athletic performance, recovery and sleep appear on none of them. In adult deficiency the label starts at about 0.2 mg a day (0.15 to 0.30 mg) and raises it by 0.1 to 0.2 mg every one to two months against IGF-1, and the Endocrine Society advises 0.1 to 0.2 mg a day for patients over 60 (Molitch 2011). The anti-aging trials started at an average of 14 mcg/kg a day (Liu 2007), about 1 mg for a 70 kg adult, roughly five times that starting replacement dose.

Why HGH for anti-aging is a federal crime

For almost every prescription drug, off-label prescribing is legal; the tesamorelin profile depends on it. Growth hormone is the exception written into the Food, Drug, and Cosmetic Act. Section 333(e) makes it a felony to knowingly distribute human growth hormone, or possess it with intent to distribute, for any use other than treating a disease or recognized condition where that use has been authorized by the Secretary of Health and Human Services under the drug-approval section of the Act (in practice, an FDA-approved indication) and ordered by a physician. The penalty is up to five years in prison, ten if the recipient is under 18; a conviction counts as a Controlled Substances Act felony for forfeiture; the DEA is authorized to investigate; and "human growth hormone" means somatrem, somatropin or an analogue of either (21 U.S.C. 333(e)).

Three consequences follow from that text.

  1. Off-label is the offense. Both conditions must be met: an authorized use and a physician's order. A prescription for anti-aging, bodybuilding or a "low-normal IGF-1" satisfies the second and fails the first.
  2. A low IGF-1 is not an indication. Adult GH deficiency is a specific diagnosis, made the way the next sections describe.
  3. Secretagogues sit outside the statute. They are analogues of GHRH or ghrelin, not of somatropin, so they are regulated like other drugs: approved, compounded under section 503A, or unapproved. That is why anti-aging clinics prescribe them instead of GH, and why the legal question for them is the compounding question in our legal tracker.

How secretagogues differ from growth hormone

There are three families, and the differences matter more than the shared label.

  • GHRH analogs bind the growth hormone-releasing hormone receptor on the pituitary: sermorelin (GHRH 1-29, gone within minutes), tesamorelin (the full 44 amino acids, stabilized) and CJC-1295 (a modified 1-29 whose DAC form binds albumin and lasts days).
  • Injectable ghrelin-receptor agonists act on GHS-R1a: ipamorelin, and older peptides such as GHRP-2 and GHRP-6.
  • Oral ghrelin-receptor agonists: MK-677, a small molecule rather than a peptide. A relative, macimorelin, is the only ghrelin-receptor agonist FDA has approved, and only as a single-dose diagnostic test for adult GH deficiency (Garcia 2018).

All of them work through the pituitary, which still answers to somatostatin and to rising IGF-1. That feedback is the real difference from injected GH, and it cuts both ways. It is a safety ceiling: a healthy pituitary will not be driven to acromegaly-range output. It is also an effect ceiling: the IGF-1 rise is smaller and less predictable, and in someone whose pituitary has been removed or irradiated, the drug has nothing to act on, which is why tesamorelin's label lists a disrupted hypothalamic-pituitary axis as a contraindication. Injected somatropin has no such limit. The dose sets the level, and the high IGF-1 it produces suppresses the pituitary's own output instead of capping the drug.

The pattern differs too. Sermorelin gives one short overnight pulse; MK-677 enhanced pulsatile GH secretion in its two-year trial (Nass 2008); CJC-1295 with DAC has a half-life of 5.8 to 8.1 days (Teichman 2006), a sustained stimulus rather than a pulse. "Physiological" describes some secretagogues, not the class.

What the evidence shows: GH versus the secretagogues

Growth hormone and the secretagogues, graded on what each has shown in humans
ClaimGradeWhat the evidence showsSources
GH treats diagnosed adult GH deficiencyAThe Endocrine Society guideline concludes that GH improves body composition, exercise capacity, skeletal integrity and quality of life in adults with confirmed deficiency, most in those with severe deficiency, at low risk, with doses individualized.Molitch 2011
GH changes body composition in healthy older adultsA18 study populations, 220 GH-treated participants, mean age 69: fat mass fell 2.1 kg and lean mass rose 2.1 kg, with no change in weight, bone density or most lipids.Liu 2007
GH improves strength, function or performance without deficiencyDIn older adults, no functional benefit was shown and edema, joint pain, carpal tunnel syndrome, gynecomastia were more common, new diabetes and impaired fasting glucose somewhat more common; the authors concluded GH cannot be recommended for anti-aging. In 11 trials of 254 healthy young adults, GH raised lean mass but not strength or aerobic capacity.Liu 2007; Hermansen 2017
Tesamorelin reduces visceral fatA806 adults with HIV, 2 mg daily for 26 weeks: visceral fat -24 versus +2 cm2 on placebo (treatment effect -15.4%), subcutaneous fat unchanged, IGF-1 up 108 ng/mL; the reduction held to 52 weeks with continued treatment.Falutz 2010
Sermorelin raises GH and IGF-1 in older adultsB10 healthy men aged about 68: 1 mg twice daily for 14 days restored 24-hour GH and IGF-1 to the levels of young men; 0.5 mg twice daily did not. Fasting glucose was unchanged.Corpas 1992
MK-677 increases lean mass in older adultsB65 adults aged 60 to 81, 25 mg daily: fat-free mass +1.1 kg versus -0.5 kg on placebo at one year, weight +2.7 versus +0.8 kg, fasting glucose up about 5 mg/dL, insulin sensitivity down, no change in visceral fat, strength or function.Nass 2008
CJC-1295 raises GH and IGF-1BTwo placebo-controlled trials of 28 and 49 days in healthy adults: one injection raised mean GH 2- to 10-fold for 6 days or more and IGF-1 1.5- to 3-fold for 9 to 11 days. No outcome was measured.Teichman 2006
A secretagogue delivers GH's benefits with fewer risksDNo trial has compared any secretagogue with GH. The largest secretagogue body-composition trial in healthy older adults added 1.1 kg of fat-free mass against a 0.5 kg loss on placebo, with no strength gain and its own glucose cost.Liu 2007; Nass 2008
A = strong human trials or meta-analyses · B = small or short human trials · C = observational, mechanistic or indirect human data · D = animal/in-vitro only, or no evidence. Grades describe the evidence, not whether we recommend the use.

Read the table in two halves. Growth hormone has the strongest evidence of an effect and the clearest evidence that, outside deficiency, the effect does not buy function and does cost side effects. In the adult-deficiency trials of one brand, peripheral swelling affected 17.5% on GH versus 5.1% on placebo in the first six months, joint pain 17.3% versus 4.2% and paresthesia 9.6% versus 1.9%, mostly early in treatment and often transient or relieved by a lower dose (Genotropin label 2026). The secretagogues have weaker evidence of an effect, and the pitch that they deliver GH's benefits more safely has never been tested.

The secretagogues, briefly

Tesamorelin is the only one with large placebo-controlled trials and a current approval, and the only one whose main claim, less visceral fat, is grade A, in HIV lipodystrophy (Falutz 2010). Outside HIV there is one 60-person trial; the fat returns when treatment stops. Details in the tesamorelin profile.

Sermorelin raises GH and, at 1 to 2 mg a day, IGF-1 in older adults (Corpas 1992), in small trials of 10 to 19 people; outcomes were small and inconsistent, and most clinics prescribe 200 to 500 mcg a night, well below the studied doses. Details in the sermorelin profile.

MK-677 has the best body-composition trial of the class and the clearest metabolic cost (Nass 2008), and it cannot be legally compounded.

CJC-1295 and ipamorelin have hormone studies, no outcome data at the doses sold, no trial of the combination and no legal supply; the four-way comparison has the full record.

IGF-1 and cancer: the caution that applies to all of them

IGF-1 is a growth and survival signal for cells, and every drug on this page raises it on purpose. In a meta-analysis of 21 studies with 3,609 cancer cases and 7,137 controls, people whose natural IGF-1 sat at the 75th rather than the 25th percentile had higher odds of prostate cancer (odds ratio 1.49) and premenopausal breast cancer (1.65); the authors called the associations modest and variable between sites (Renehan 2004). That is observational data about naturally high IGF-1, not proof that a drug-induced rise causes cancer. It is still the reason the labels are strict.

Somatropin is contraindicated in active malignancy; any earlier cancer must be inactive and its treatment complete, the drug must stop if it recurs, and childhood cancer survivors treated with head radiation who later received GH had more second tumors, mostly meningiomas (Genotropin label 2026). Tesamorelin carries the same active-malignancy contraindication. For the secretagogues the honest answer is that nobody knows: the longest controlled exposure is two years in 65 people, too small and short to count cancers. The practical rules are the same for all of them: cancer screening up to date before starting, IGF-1 kept within the age-adjusted range, and no start at all after a cancer diagnosis without the oncologist.

How adult GH deficiency is actually diagnosed

A legitimate GH prescription starts with this, and most wellness intakes skip it.

  • Who gets tested. Adults with a reason to suspect deficiency: pituitary or hypothalamic disease, pituitary surgery or radiation, head trauma, or GH deficiency in childhood, the causes the label lists (Genotropin label 2026). Feeling older is not on the list.
  • Stimulation testing is usually required. Confirmation through stimulation testing is needed unless there is a proven genetic or structural lesion persisting from childhood (Molitch 2011). The Endocrine Society names the insulin tolerance test and the GHRH-arginine test as accurate enough to establish the diagnosis, with peak GH cut-points of 5.1 and 4.1 mcg/L, the latter adjusted for body mass index; the glucagon test, with a cut-point of 2.5 to 3 mcg/L, is the fallback when GHRH is unavailable and the insulin test is unsafe or impractical. With GHRH no longer marketed in the US, the practical choices are the insulin tolerance test, the glucagon test and an oral one.
  • The oral test. Macimorelin, taken as a single oral dose, was compared with the insulin tolerance test in 139 people: using a GH cut-off of 2.8 ng/mL it had 87% sensitivity and 96% specificity, and gave the same answer on retesting 97% of the time (Garcia 2018).
  • What IGF-1 can and cannot do. A normal IGF-1 does not exclude deficiency, and a low one, absent a catabolic state such as serious illness or malnutrition, is strong evidence for it. Deficiencies in three or more other pituitary hormones make GH deficiency likely enough that testing becomes optional (Molitch 2011).

The consequence for anyone shopping online: a questionnaire plus a single IGF-1 cannot legitimately produce a growth hormone prescription. It can, legally, lead to a sermorelin or tesamorelin prescription, which is exactly the gap the HGH-peptide market fills. Our guide to who qualifies for peptide therapy covers the rest of that evaluation.

Which to choose, by situation

You have pituitary disease, pituitary surgery or radiation, or childhood-onset GH deficiency. See an endocrinologist for testing. If deficiency is confirmed, somatropin is the treatment, usually covered by insurance; secretagogues are a poor substitute because they need the pituitary that is damaged.

You have HIV with abdominal fat or wasting. Tesamorelin for visceral fat and Serostim for wasting are the approved drugs, prescribed by the HIV clinician.

Your visceral fat is out of proportion to your weight. Tesamorelin, off-label, on the strength of one trial outside HIV, with IGF-1, glucose and the fat itself measured. If you are overweight overall, GLP-1 therapy has far larger trials; see best peptides for weight loss.

You are a symptomatic adult with a low IGF-1 and no pituitary disease. Sermorelin is the lowest-cost prescribable option with feedback intact. Ask what dose the clinician expects to move IGF-1, and when it will be measured; the sermorelin dosage chart has the arithmetic.

You want muscle, strength or performance. None of these. GH itself did not improve strength in healthy young adults (Hermansen 2017), and all are prohibited in tested sport. The muscle-growth guide covers what does work.

You want to slow aging. None has evidence for it; see best peptides for longevity.

Who should not use any of them

The labels for somatropin and tesamorelin, and the physiology they share with every secretagogue, give one list:

  • active cancer, or a cancer history the prescriber and oncologist have not reviewed;
  • active proliferative or severe non-proliferative diabetic retinopathy (a somatropin contraindication), and diabetes or prediabetes without close glucose monitoring, since GH and MK-677 both reduce insulin sensitivity;
  • acute critical illness after major surgery, trauma or respiratory failure, where pharmacologic GH increased mortality (Genotropin label 2026);
  • pregnancy or breastfeeding (a tesamorelin contraindication; no data for the others);
  • an unevaluated pituitary or brain mass: GH deficiency can be the first sign of a pituitary tumor, which has to be ruled out before GH is started (Genotropin label 2026);
  • children and teenagers outside a pediatric endocrinology clinic;
  • athletes subject to testing, and anyone unwilling to have IGF-1 and glucose measured before and after.

Bottom line

Growth hormone works, for the people it is approved for, and is a federal crime to distribute for anyone else. In healthy older adults it changed body composition by about 2 kg in each direction and changed nothing they could do, at the price of swelling, joint pain and higher glucose. The HGH peptides exist because of that law, not because they were shown to be a safer route to the same result; no trial has compared them with GH. Two of them can be prescribed. Tesamorelin has real evidence for visceral fat in a specific population. Sermorelin is a cheap, feedback-preserving nudge with small old trials and a dose gap. The other three have no legal supply. If you think you might have GH deficiency, the step that matters is a stimulation test, not a peptide.

Frequently asked questions

+Are HGH peptides the same as HGH?

No. HGH is somatropin, a 191-amino-acid recombinant copy of growth hormone that replaces the hormone directly. 'HGH peptides' are secretagogues such as sermorelin, tesamorelin, CJC-1295 and ipamorelin, plus the oral non-peptide MK-677: they signal your own pituitary to release growth hormone, so the body's feedback loop still caps the response and they need a working pituitary. They are regulated differently too: distributing somatropin for anything other than an FDA-authorized use is a federal felony, while secretagogues are ordinary drugs that are either approved, compoundable or unapproved.

+What peptides increase HGH?

Two groups. GHRH analogs act on the growth hormone-releasing hormone receptor: sermorelin, tesamorelin and CJC-1295. Ghrelin-receptor agonists act on the GHS-R1a receptor: ipamorelin, older peptides such as GHRP-2 and GHRP-6, and the oral molecule MK-677, which is not a peptide. All of them raise growth hormone in human studies. Only tesamorelin and sermorelin can be legally prescribed in the United States in October 2026: tesamorelin as the brand Egrifta, which cannot be compounded, and sermorelin as a compounded drug.

+Do growth hormone peptides work?

They raise growth hormone and, at adequate doses, IGF-1; that part is documented. Whether that changes anything you would notice is a different question. Tesamorelin cut visceral fat about 15% over 26 weeks in 806 adults with HIV. MK-677 added about 1 kg of fat-free mass over a year in older adults with no gain in strength. Sermorelin, CJC-1295 and ipamorelin have hormone data and little or nothing on outcomes. No secretagogue has been shown to slow aging, improve recovery or build strength.

+Is it legal to get HGH for anti-aging?

No. Under 21 U.S.C. 333(e), knowingly distributing human growth hormone, or possessing it with intent to distribute, for any use other than one FDA has authorized and a physician has ordered is a felony punishable by up to five years in prison, or ten if the recipient is under 18. Aging, bodybuilding and athletic performance are not authorized uses, and a doctor's order alone does not change that. Growth hormone is approved for diagnosed GH deficiency in adults and children, several pediatric growth disorders and HIV wasting.

+Which HGH peptides can be prescribed in the US?

Tesamorelin, which is FDA-approved as Egrifta for HIV-associated abdominal fat and may be prescribed off-label as the brand (it is a biological product, so no pharmacy may compound it), and sermorelin, a formerly approved drug that licensed 503A pharmacies may compound on prescription. CJC-1295, ipamorelin and MK-677 cannot be compounded by any licensed US pharmacy as of October 2026; what is sold online under those names is a research chemical.

+Is MK-677 an HGH peptide?

It is sold as one but it is not a peptide: ibutamoren mesylate is an oral small molecule that activates the ghrelin receptor. Its two-year trial in 65 adults aged 60 to 81 raised growth hormone and IGF-1 to young-adult levels and fat-free mass by about 1 kg, with weight gain, higher fasting glucose and no improvement in strength or function. FDA has kept it in Category 2 since September 2023, so no pharmacy may compound it.

+How do I know if I have growth hormone deficiency?

Adult GH deficiency is diagnosed by an endocrinologist, usually in someone with pituitary or hypothalamic disease, surgery, radiation, head trauma or childhood-onset deficiency. It normally requires a stimulation test, such as the insulin tolerance test, the glucagon test or the oral macimorelin test, because a normal IGF-1 does not rule it out and a low-normal one does not prove it. Feeling tired at 50 with an IGF-1 at the bottom of the lab range is not a diagnosis.

+Do HGH peptides cause cancer?

Nobody knows, because no secretagogue trial has been large or long enough to count cancers; the longest ran two years in 65 people. The concern is real enough to act on: in pooled observational studies, people with naturally higher IGF-1 had modestly higher rates of prostate and premenopausal breast cancer, and every drug on this page raises IGF-1. Somatropin and tesamorelin are both contraindicated in active cancer. Anyone with a cancer history should not start one without their oncologist.

+How much does HGH peptide therapy cost?

In October 2026, compounded sermorelin runs about $150 to $400 a month and brand Egrifta, the only legal tesamorelin, $2,400 to $2,800. Baseline labs add $100 to $400. Somatropin itself ranges from about $350 to more than $3,000 a month cash depending on brand and dose, and is usually covered by insurance only with a confirmed diagnosis. CJC-1295, ipamorelin and MK-677 have no legitimate price because no licensed pharmacy may make them.
