Peptides for Weight Loss in Women: What Works After 40
Peptides for weight loss in women after 40: what the GLP-1 trials show in women and after menopause, why women lose more than men, the birth control and pregnancy rules on the labels, muscle and bone, hormone therapy, the peptides with no data, and October 2026 costs.
Published October 12, 2026 · 8 min read
Key takeaways
- The peptides that work for weight loss in women after 40 are the prescription GLP-1 drugs, tirzepatide and semaglutide. Their pivotal trials were mostly women in their mid-40s: 74% of STEP 1 and 68% of SURMOUNT-1.
- They work after menopause. In a SURMOUNT analysis, postmenopausal women on tirzepatide lost 23% of their body weight against 3% on placebo, and their waists shrank by about 20 cm. On average, women lose more weight than men on GLP-1 drugs.
- Tirzepatide cuts the absorption of birth control pills after the first dose and each dose increase: the label says to switch to a non-oral method or add a barrier method for 4 weeks each time. Both drugs must be stopped for pregnancy, Wegovy at least 2 months before trying.
- About a quarter of the weight lost on tirzepatide is lean mass. In one trial, a GLP-1 drug alone lowered hip and spine bone density more than exercise, while the two together preserved it. Resistance training and protein are part of the treatment, not extras.
- AOD-9604, MOTS-c, 5-amino-1MQ and CJC-1295 with ipamorelin, sold as women's fat-loss peptides, have no human weight-loss data and no legal compounded source in October 2026.
The peptides for weight loss in women that have real evidence are the GLP-1 drugs: tirzepatide (Zepbound) and semaglutide (Wegovy). They were tested mostly in women in their 40s, they work after menopause, and on average women lose more weight on them than men do. What changes after 40 is the fine print: contraception in perimenopause, lean mass and bone, and how they sit alongside hormone therapy. This page covers those, and the parent guide, best peptides for women over 40 and 50, ranks the options for skin, sleep and energy. The companion page on injectable peptides for anti-aging covers the rest of what clinics inject.
What the trials show in women
| Evidence | Who was studied | What it found | Grade |
|---|---|---|---|
| STEP 1: semaglutide 2.4 mg weekly, 68 weeks | 1,961 adults without diabetes; 74% women; mean age 46 | 14.9% weight loss vs 2.4% on placebo; 86% lost at least 5% (Wilding 2021) | A |
| SURMOUNT-1 by reproductive stage: tirzepatide 15 mg or maximum tolerated dose, 72 weeks | Women from a 2,539-adult trial that was 68% women, mean age 45 | Weight loss 26% premenopause, 23% perimenopause, 23% postmenopause, vs 2% to 3% on placebo; waist down 20 to 22 cm vs 4 to 5 cm (Tchang 2025) | A for the trial; B for the post hoc subgroups |
| Sex differences across GLP-1 trials | Meta-analysis of 14 randomized trials | Women lost about 1 kg, or 1.7 percentage points, more than men (Yang 2025) | B |
| Body composition with tirzepatide | 160-person DXA substudy, 73% women | About 75% of weight lost was fat, 25% lean mass, consistent across most subgroups including sex and age (Look 2025) | B |
| Bone with a GLP-1 drug, with and without exercise | 195 adults, 64% women, mean age 43 | Liraglutide alone lowered hip and spine density more than exercise; the combination preserved it (Jensen 2024) | B |
| Peptides sold for female fat loss: AOD-9604, MOTS-c, 5-amino-1MQ, CJC-1295 with ipamorelin | No trials in women | No human weight-loss evidence | D |
Female shares and ages are from the ClinicalTrials.gov results records (STEP 1, SURMOUNT-1); side-effect tables are in compounded semaglutide versus tirzepatide.
After 40, and after menopause
The trials did not need to be translated to women; women were most of the participants. STEP 1 enrolled 1,961 adults with obesity, or overweight with a weight-related condition, and 68 weeks of semaglutide 2.4 mg a week produced a mean loss of 14.9% against 2.4% on placebo, with half of participants losing at least 15% and 4.5% stopping for gut side effects (Wilding 2021).
Menopause is the next question, because fat shifts toward the abdomen around the final period. A post hoc analysis of three SURMOUNT trials sorted women by reproductive stage and found the same response at every stage: 97% to 98% of women on tirzepatide lost at least 5% of their weight, against 29% to 33% on placebo (Tchang 2025). The analysis was written mostly by Lilly employees and the stages were assigned after the trial, which is why we grade the subgroup result B. For semaglutide, the Spanish Menopause Society's 2026 position statement calls the menopause-specific evidence the largest of any incretin drug but "limited and largely observational" (Sánchez-Prieto 2026).
Women also tend to lose more than men. Across 14 randomized trials of GLP-1 drugs, women lost on average 1.04 kg more, and the gap grew with the size of the weight loss and in trials run for obesity rather than diabetes (Yang 2025). Tirzepatide was not in that analysis.
Contraception, pregnancy and breastfeeding
Women in their 40s can still conceive, and irregular perimenopausal cycles are not contraception.
- The pill and tirzepatide. Tirzepatide slows stomach emptying. After a single 5 mg dose given with a combined pill, peak ethinyl estradiol fell 59% and total exposure 20%; the effect is largest after the first dose and fades. The label advises a non-oral method, or an added barrier method, for 4 weeks after starting and for 4 weeks after each dose increase. Non-oral hormonal methods are not expected to be affected (Zepbound label).
- The pill and semaglutide. The Wegovy label reports no clinically significant change in ethinyl estradiol or levonorgestrel exposure with semaglutide (Wegovy label).
- Pregnancy. Both labels say weight loss offers no benefit in pregnancy and may harm the fetus. Zepbound is stopped when a pregnancy is recognized; Wegovy should be stopped at least 2 months before a planned pregnancy because of semaglutide's long half-life. Both companies run pregnancy exposure registries.
- Breastfeeding. In a single-dose study in 11 lactating women, tirzepatide in breast milk was undetectable or low, with no infant data. Breastfeeding is not recommended with Wegovy tablets, and there are no milk data for the injection.
- Hair. In the Zepbound trials, hair loss was reported by 7.1% of women against 0.5% of men, and the label links it to weight reduction. It is usually temporary shedding; see does tirzepatide cause hair loss.
Muscle, bone and hormone therapy
Lean mass. In 160 SURMOUNT-1 participants scanned by DXA, 73% of them women, tirzepatide reduced weight by 21.3%, fat mass by 33.9% and lean mass by 10.9% at 72 weeks; about three-quarters of the weight lost was fat in both the drug and placebo groups, and the proportion stayed consistent across most subgroups, which included sex and three age bands up to 65 and older (Look 2025). Lean mass includes water and organs as well as muscle, but it is worth protecting after 50.
Bone. In a Danish trial of 195 adults, 64% women, who first lost weight on an 800-calorie diet, a year of liraglutide 3.0 mg alone lowered bone density at the hip and spine more than exercise alone despite similar weight loss, while liraglutide plus exercise produced the most weight loss and kept bone density level with placebo (Jensen 2024). Liraglutide is an older GLP-1 drug, and the finding has not been tested with semaglutide or tirzepatide, but the message for a woman near menopause is practical: the drug and the weights belong together.
Hormone therapy. The Spanish Menopause Society recommends that GLP-1 treatment in menopause include resistance exercise, adequate protein, attention to fracture risk and monitoring of function, and warns that the link between hormone therapy and greater semaglutide weight loss is hypothesis-generating and should not be the reason to start hormones (Sánchez-Prieto 2026). No study has tested either drug with menopausal estradiol tablets; patches and gels bypass the stomach. The detail is in peptides for menopause.
What does not work
AOD-9604 failed its human trials, 5-amino-1MQ is a small molecule with mouse data, MOTS-c has mouse data and a July 2026 committee vote, and CJC-1295 with ipamorelin has no weight data; none can be legally compounded in October 2026 (legal tracker). Sermorelin is legal but has no weight-loss trial. Tesamorelin reduces visceral fat in people with HIV, mostly men, and has not been tested in menopausal women. The full ranking is in best peptides for weight loss.
Doses and units
Wegovy starts at 0.25 mg weekly and steps up every 4 weeks through 0.5, 1 and 1.7 mg to a 1.7 or 2.4 mg maintenance dose, with up to 7.2 mg for selected patients (Wegovy label). Zepbound starts at 2.5 mg weekly for 4 weeks and rises in 2.5 mg steps at least 4 weeks apart to a 5, 10 or 15 mg maintenance dose (Zepbound label). Brand pens deliver fixed doses. A compounded vial is drawn up in insulin-syringe units, where units equal the dose in mg divided by the concentration in mg per mL, times 100: 5 mg of tirzepatide from a 10 mg per mL vial is 50 units, and 0.25 mg of semaglutide from a 2.5 mg per mL vial is 10 units. Check your own label's concentration in the peptide calculator or the units converter; the full charts are in the tirzepatide and semaglutide dosage pages.
What it costs in October 2026
- Wegovy: $349 a month cash through Novo Nordisk's pharmacy, $199 for the first two fills for new patients in 2026; the Wegovy pill costs $149 to $299 by dose.
- Zepbound: $299 at 2.5 mg, $399 at 5 mg and $449 at 7.5 to 15 mg through Lilly's self-pay program with on-time refills; about $1,147 in medication for the first three months.
- Compounded: semaglutide $150 to $350 and tirzepatide $180 to $300 a month, legal only for a documented individual need since the shortages ended.
- Medicare: since July 1, 2026, eligible Part D enrollees can get the Zepbound KwikPen at a $50 copay through the temporary GLP-1 Bridge.

Bottom line
For women after 40, the peptides that work for weight loss are tirzepatide and semaglutide, after menopause as well as before. What is specific to women is practical: backup contraception during tirzepatide dose steps, stopping before pregnancy, resistance training and protein for muscle and bone, and one clinician who sees the hormone and weight plans together. Every cheaper 'women's fat-loss peptide' has no trial behind it.
Frequently asked questions
+What is the best peptide for female fat loss?
Tirzepatide, followed closely by semaglutide. In a SURMOUNT analysis of women only, tirzepatide at 15 mg or the highest tolerated dose produced 23% to 26% weight loss against 2% to 3% on placebo, whatever the woman's menopausal stage. Semaglutide 2.4 mg produced 14.9% against 2.4% in STEP 1, a trial that was 74% women. Both are FDA-approved; compounded copies are legal only for a documented individual need. No other peptide sold for fat loss has comparable human data in women.
+Do GLP-1 drugs work after menopause?
Yes. In the SURMOUNT-1 analysis by reproductive stage, postmenopausal women on tirzepatide lost 23% of their body weight against 3% on placebo, the same as perimenopausal women and close to the 26% seen before menopause, and their waists shrank by about 20 cm. For semaglutide, the menopause-specific evidence is smaller and mostly observational, according to a 2026 Spanish Menopause Society position statement.
+Do women lose more weight than men on GLP-1 drugs?
On average, yes. A 2025 meta-analysis of 14 randomized trials found that women lost about 1 kg, or 1.7 percentage points of body weight, more than men, and the gap widened as total weight loss grew and in trials run for obesity. The analysis covered semaglutide, liraglutide, dulaglutide, exenatide and retatrutide but not tirzepatide.
+Does tirzepatide affect birth control?
Yes, for pills. After a single 5 mg dose given with a combined oral contraceptive, peak ethinyl estradiol fell 59% and total exposure 20%, because tirzepatide slows stomach emptying. The Zepbound label advises switching to a non-oral method, or adding a barrier method, for 4 weeks after starting and for 4 weeks after each dose increase. According to the label, non-oral hormonal methods should not be affected. The semaglutide label reports no clinically significant change in ethinyl estradiol or levonorgestrel levels.
+Can I take semaglutide or tirzepatide if I am trying to get pregnant or breastfeeding?
Not while trying to conceive. Both labels say weight loss offers no benefit in pregnancy and may harm the fetus; Zepbound should be stopped when a pregnancy is recognized, and Wegovy should be stopped at least 2 months before a planned pregnancy because semaglutide stays in the body for weeks. In a single-dose study in 11 breastfeeding women, tirzepatide was undetectable or low in breast milk, with no data on infants; breastfeeding is not recommended with Wegovy tablets, and there are no milk data for the injection.
+Will I lose muscle or bone on a GLP-1?
Some lean mass, yes: in a DXA substudy of SURMOUNT-1 (73% women), about 25% of the weight lost on tirzepatide was lean mass and 75% fat, the same split as on placebo, and it stayed consistent across most subgroups, including sex and age. Bone is less certain. In a 52-week trial after diet-induced weight loss, liraglutide alone lowered hip and spine bone density more than exercise alone, while liraglutide plus exercise preserved it. Resistance training and enough protein are the standard safeguards.
+Is there an over-the-counter peptide for female fat loss?
No. Every peptide with weight-loss evidence is a prescription drug. Products sold without a prescription are either supplements, such as collagen peptides, which are food and have no weight-loss trials of this kind, or research vials of compounds such as AOD-9604, MOTS-c or 5-amino-1MQ, which have no human weight-loss data in women and cannot be legally compounded by a US pharmacy in October 2026.
+What is the best peptide for belly fat loss in women?
A GLP-1 drug, because it shrinks the waist by shrinking total weight: in the SURMOUNT-1 analysis, women's waist circumference fell 20 to 22 cm on tirzepatide against 4 to 5 cm on placebo. Tesamorelin reduces visceral fat specifically, but its trials were in people with HIV, mostly men, and it has not been tested in menopausal women. No peptide targets menopausal belly fat on its own.
